29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) Pelletizer #003; On or about January 10, 2019 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to work on the pelletizer(s) to clean and clear jams on the cutter without isolating the machine from the energy source - employees were relying on a safety interlock switch versus unplugging the machine to isolate the machine from the energy source.
