29 CFR 1926.416(a)(3): Before work was begun the employer did not ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work may bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer did not post and maintain proper warning signs where such a circuit exists. The employer did not advise employees of the location of such lines, the hazards involved, and the protective measures to be taken. a) 3475 Piedmont Rd, Atlanta (Construction site) - On or about 1/7/2019, the employer exposed employees to electrical shock and arch flash hazards in that the employer did not post and maintain proper warning signs for underground power lines and neither advised employees of the location of such lines, the hazards involved, and the protective measures to be taken while employees were inside an open excavation cutting and removing three underground power lines to install a new 8 inch ductile iron pipe water supply line inside the trench.
