29 CFR 1915.181(b): Before an employee was permitted to work on an electrical circuit, except when the circuit must remain energized for testing and adjusting, the circuit was not de-energized and checked at the point at which the work was done to ensure that it was actually de-energized: USS Miguel Keith, STBD Casing 1-34-1: On November 28, 2018, employees were performing a continuity check on a panel containing energized components. An employee contacted two different phases of the line side concurrently with a set of alligator clips, causing an arc flash. The employee was exposed to electrical shock and/or burn hazards from 480 volts.
