29 CFR 1910.333(b)(2)(ii)(A): Safe procedures for deenergizing circuits and equipment were not determined before circuits and equipment were de-energized. On or about October 25, 2018, at the Oryx Lease/Pyote Truck Station off Hwy 2355, employees were exposed to electrical arc flash hazards while reconnecting electrical cables in a VFD Electrical Building without properly verifying that all components had been deenergized.
