29 CFR 1910.28(b)(3)(iv): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. On or about October 29, 2018, and times prior thereto: a. In the Weld Shop Area, where employees were exposed to a fall hazard when changing filters on Furnace G-339 without a self-closing gate or offset on the opened end of the ladderway platform. b. In the Weld Shop Area, where employees were exposed to a fall hazard when changing filters on Furnace G-340 without a self-closing gate or offset on the opened end of the ladderway platform. c. In the Test Lab Prep Area, where employees were exposed to a fall hazard when changing filters on Furnace H-124 without a self-closing gate or offset on the opened end of the ladderway platform. d. In the Test Lab Prep Area, where employees were exposed to a fall hazard when changing filters on Furnace H-105 without a self-closing gate or offset on the opened end of the ladderway platform.
