29 CFR 1926.1053(b)(15): Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could have affected their safe use: On or about October 25, 2018, an Orange 35 foot Extension Ladder was taken apart and used without the slip resistant feet on the ladder and had not been inspected prior to use, exposing employees to a fall hazard of approximately 15 feet.
