29 CFR 1910.146(j)(2): The employer did not ensure that each entry supervisor verified, by checking that the appropriate entries had been made on the permit, that all tests specified by the permit had been conducted and that all procedures and equipment specified by the permit were in place before endorsing the permit and allowing entry to begin: a) 52-1 pit, on or about 6/30/18 through 7/6/18: Entries into the 52-1 water pit on 6/30/18, 7/3/18, 7/5/18 and 7/6/18, were allowed to proceed without the use of retrieval devices and harnesses. The permits did not include a check mark for applicable retrieval devices including full body harnesses. Abatement certification must be submitted for this item
