29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) CANISTER ROOM - The machine specific lock out tag out procedure for de-energizing the peel collection canisters in the canister room did not contain required information, including all of the energy sources required to be controlled; the specific procedural steps for shutting down, isolating, blocking and securing the equipment to control hazardous energy; the specific procedural steps for the placement, removal and transfer of lockout or tagout devices and the responsibility for them; the specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures, on or about October 2, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
