29 CFR 1926.950(c)(2)(i): The employer did not ensure that each employee was instructed in the hazardous conditions relevant to the employee's work that the employer was aware of as a result of the information communicated to the employer by the host employer under section (c)(1) of this section. a) On September 12, 2018 at 211 Greenville Road, sub-station, Greenville AL: The employer failed to ensure that employees were instructed in the hazardous conditions relevant to the work the employer was aware of as a result of the information communicated by the host employer (Alabama Power). The employer was aware that 115kv lines being fed from two different sub-stations, Greenville and Evergreen, which connected and looped through the substation at 211 Greenville Road, Greenville, Al. Supervision failed to communicate the potential hazardous condition of connecting the temporary shoofly lines in that they were dealing with two separate feeds of 115kv and if the switches were opened and cut before the shoofly was connected that would create a different load potential and a hazard of arc flash when phases were connected.
