29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: 14 Mechanic St, Southbridge, MA: On September 18, 2018 and at times prior, employees performing maintenance and servicing tasks periodically shared a single lockout device for the protection of multiple employees. On September 18, 2018, two maintenance employees who were exposed to hazards while working on the Chicago Ironing Folding machine placed only one lock on the electrical energy source.
