29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures: a) Facility, 200 Utah Street, Harmony, PA 16037, On or before September 11, 2018, the employer did not train the Cincinnati Shear operators to an authorized level under the locations hazardous energy isolation control program. The operators remove multiple stuck/jammed pieces from the shear throughout a shift.
