29 CFR 1926.301(a): Unsafe hand tool(s) were issued or permitted to be used: On or about August 17, 2018, employees at the Shaft #3 H-08 regulator shaft were using a damaged portable electrical Enerpac hydraulic pump unit (model PUJ1200B) and hydraulic single acting cylinder ram (Model RC104). The Enerpac electric start switch was broken off, the unit identification and capacity label were missing and the cylinder ram was leaking hydraulic oil.
