29 CFR 1926.502(b)(1): The top edge height of top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a) HVAC Duct Floor Openings on 2nd, 3rd and 4th Floors - On or about August 15, 2018 and at times prior to, the top edge of a top rail installed on an HVAC duct floor opening was not at 42 inches plus or minus 3 inches above the walking/working level, thus exposing employees to fall hazards of up to approximately 30 feet.
