29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5): The employer is failing to protect employees from amputation and struck-by hazards. This was most recently documented on or about 08/03/18 at 1351 S. Reca Ct, Wichita, Kansas, where the forklift operator and employees near the forklift(s) were exposed in that the forklift operator had not been provided both formal and practical powered industrial truck training prior to operating the forklift(s).
