29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about July 24, 2018 and at times prior there to, the employer did not ensure lockout or tagout devices were affixed to each energy isolating device by authorized employees before an employee began to exchange blades on a planer. This exposed employees to amputation hazards.
