29 CFR 1926.417(b): Equipment or circuits that were de-energized were not rendered inoperative, and did not have tags attached at all points where such equipment or circuits could be energized: a) On or about July 26, 2018 at the location of 5120 Advantage Drive, Toledo OH: the employer did not ensure employees were protected from electrical hazards when removing and installing lighting fixtures. Employees were not tagging out the circuits on the breaker panels while performing these tasks.
