On or about July 18, 2018 at the facility located at 2685 Universal St Oshkosh, WI 54903, the employer did not properly guard the point of operation of the L&J Press Corporation Mechanical Power Press serial number 33217. The fixed barrier guard was not securely attached to the frame of the press.
