Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Facility, Assembly area, 71 Progress Avenue, Cranberry Township, PA: On or before July 18, 2018 an energy control device was not available or placed on the switch on the tilt table. The airline had been removed by from the table by an employee who was to clean in and around the machine and bander. Unbeknown to the employee working at the table another employee turned the switch for the table on and when it didn't function, walked away. The employee who was cleaning, finished and reattached the airline to the table. The table immediately began to lift and trapped the employee between the table and the floor. The switch to the table had not been locked or tagged to isolate the machine and control the energy.
