29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers. a. On or about June 29, 2018, at the above addressed jobsite, an employee was conducting asbestos removal operations in a mechanical room exposed to an injury from stepping into unprotected floor holes (trenches) measuring eighteen (18") inches in depth by twelve (12") inches in width.
