29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Jobsite: The employer did not ensure that guardrails were installed along all open sides and ends of platforms prior to allowing the employees to use the scaffold to apply the stucco finish onto the exterior wall of the building. On July 5, 2018, one of employees fell off the end of a side bracket platform on the third level and sustained fatal injuries from the incident.
