29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of personal fall arrest systems: (a) Superior Oilfield Services Co., Ltd dba Laser Oilfield Services at 18th St. and Hwy 34, Greeley, CO 80631: On and before June 19, 2018, the employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of personal fall arrest systems. Employees working to disassemble a tank battery were not trained to understand the proper setup, anchorage, and use of a personal fall arrest system. This condition exposed employees to a fall hazard of approximately 22 feet.
