29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: a) Crown Well Services LLC at Crossroads and County Road 15 Windsor, CO 80550: On or before June 07, 2018 the employer did not ensure that a PPE Hazard Assessment had been performed to determine the appropriate PPE employees need to wear when performing completions work on a workover rig. This exposes employees to inhalation hazards related to hydrogen sulfide gas and other hazardous chemicals.
