29 CFR 1910.146(c)(5)(ii)(E)(1): An employee may not enter the space until the forced air ventilation has eliminated any hazardous atmosphere; a. On or about May 25, 2018, an employee cleaning the inside of a semi-truck tanker was pronounced dead following a confined space rescue. The employee was found to have suffered chemical asphyxia by vitiated atmosphere, hydrogen sulfide and hydrogen cyanide gases. Forced air ventilation had not eliminated the hazardous atmosphere prior to entry into the space.
