29 CFR 1910.217(d)(9)(iv): Safety blocks were not used when mechanical power press die(s) were being adjusted or repaired in the press: (a) Production - On or about May 16, 2018, the employer did not ensure that safety blocks were used to block the top die from failure when adjusting dies during set-up. An employee suffered a hand amputation after the unsecured die fell when performing set up on the Verson #58 mechanical press. (b) Production - On May 25, 2018, the employer did not ensure that safety blocks were used to block the top ram from failure when adjusting dies during set-up on the Verson #58 mechanical press.
