29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A.) Better Baked Foods, Inc.; bread shoer - On or about 5/16/18, and at times prior thereto, the bread shoer was not locked out prior to cleaning. An employee sustained a broken wrist and an amputation while cleaning the bread shoer while it was still operating.
