29 CFR 1926.601(b)(14): All vehicles in use were not checked at the beginning of each shift to assure that the following parts, equipment, and accessories were in safe operating condition and free of apparent damage which could cause failure while in use: service brakes, including trailer brake connections; parking system (hand brake); emergency stopping system (brakes); tires; horn; steering mechanism; coupling devices; seat belts; operating controls; and safety devices: (a) I-64 Eastbound Lane Closure: On May 11, 2018 and times prior, a Chevrolet C4500 truck (VIN# 1GBE4C1244F513531) that had an obstructed view to the read was operated in the workzone that was being operated in reverse without the use of an operable reverse signal alarm. The vehicle was not inspected at the beginning of each shift to ensure that safety devices such as, reverse signal alarms were operable.
