29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence. a. Toledo Tool & Die Company, Inc. located in Toledo, Ohio: On or about March 23, 2018, the employer did not ensure an employee performing die setting on the 150 ton Aida press #81 located in the production area, locked out the equipment prior to performing the task. The employee was exposed to crushing injuries between the upper and lower dies when performing the work. b. Toledo Tool & Die Company, Inc. located in Toledo, Ohio: On or about March 23, 2018, the employer did not ensure an employee performing a steel coil change over on the 250 ton Komatsu press #90 located in the production area, locked out the equipment prior to feeding steel through the die area. The employee was exposed to crushing injuries between the upper and lower dies when performing the work.
