29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) An evolution of each PlT operator performance was not being conducted at least once every three years when on or about May 29, 2018, the employer did not ensure that employees required to operate powered industrial trucks, were re-evaluated at least once every three years. The U.S. Postal Service was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 917188, citation number 1, item number 1 and was affirmed as a final order on November 13, 2013, with respect to a workplace located at 200 N. Diamond Street, Mansfield, OH 44901. The U.S. Postal Service was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 958919, citation number 1, item number 1 and was affirmed as a final order on May 13, 2014, with respect to a workplace located at 2310 Maynard H. Jackson Blvd, Atlanta GA 30320.
