29 CFR 1910.119(e)(3)(v): The process hazard analysis did not address the hazards of facility siting: (a) Dianal America Inc facility: On February 12 2018 and at times prior to, the employer failed to evaluate and address the hazards associated with locating the control room within 12 feet of process equipment/reactors that contain highly flammable chemicals. This condition exposed employees to explosion hazards.
