29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. (a) At the site located at 6004 28th Street E Bradenton, Florida employees operating self propelled scissor lift were exposed to hazards such as but not limited to, falls and/or struck- by where the employer failed to ensure that they received training, on or about February 12, 2018.
