29 CFR 1910.29(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: The employer is failing to protect employees from fall hazards of 8' 2" when working from catwalks. This was most recently documented at 2314 E Hwy 30, Grand Island, NE 68802 inside service bay 1 on or about February 6, 2018 and at times prior when employees accessed the east and west catwalks which had guardrail systems that were not equipped with either midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members installed that meet the requirements of 1910.29(b)(2)(I) to 1910.29(b)(2)(iv).
