29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities. (a) In the Polypropylene Unit of its Battleground Manufacturing Complex in LaPorte, Texas, the employer did not review operating procedures as often as necessary to ensure that the procedures currently being implemented at the facility reflect the written and certified operating procedure for upset conditions when the PDS2 Line 2 piping becomes plugged.
