29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: Location - 240 Canal St. Lawrence, Ma, 01840, 7th floor On or about December 13, 2017, an employee fell from 6ft baker scaffold due to movement of the scaffold. One of the swivel locks had not been locked to prevent movement of the scaffold while in use.
