Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1910.134(h)(4): Respirators that failed an inspection or were otherwise found to be defective were not removed from service and discarded or repaired: a) Pinnacle Powder Coating, Inc. located at 61 Saint Marys Street in Norwalk, Ohio; On or about March 28, 2018 the employer did not ensure respirators that are found to be defective are removed from service. The elastic head band of a tight fitting elastomeric half masked respirator worn by an employee was found to be in poor condition in that the elastic head strap no longer held its elasticity, inhalation valves did not match each other, and the inside surface of the respirator was covered in black material. The employee was exposed to hazards associated with exposures to particulates not otherwise regulated containing 1,3,5-Triglycidyl Isocyanurate while applying powder paint such as, but not limited to, irritation to the eyes, throat, skin, and upper respiratory system, hematologic effects, and male reproductive damage.
