All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about December 5, 2017, and on dates prior, employee(s) were not using energy isolating devices to isolate spiral conveyors from energy source(s) prior to performing inspections and cleaning on this equipment. The list of spiral conveyors includes, but is not limited to, the AmbaFlex Spiralveyor SV (Serial No. 60249-05).
