29 CFR 1910.217(d)(9)(iv): Safety blocks were not used when mechanical power press die(s) were being adjusted or repaired in the press: a.) On or about September 21, 2017, the employer did not ensure the use of die safety blocks while employees were setting up dies on the Verson BLK2 part revolution mechanical power press Employees were exposed to amputation hazards while setting up dies in the press. Maumee Assembly and Stamping, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 217(d)(9)(I) which was contained in OSHA inspection number 1223334, citation number 1, item number 3b and was affirmed as a final order on August 2, 2017 with respect to a workplace located at 920 Illinois Ave. Maumee, OH 43537.
