29 CFR 1928.57(a)(6)(v): At the time of initial assignment and annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or will be involved, including locking out electrical power before performing maintenance on farmstead equipment: (a) Manure Shed, Aisle 6, House D, Manheim Production - Not all employees who serviced farmstead equipment, such as adjusting chain tension on the chain and sprocket drives of the manure belts, had received training to lockout the electrical power before performing this servicing and so did not lockout the electrical power, exposing employees to getting caught in or caught by moving machine parts, on or about August 23, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
