29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about 8/31/2017, and at times before, the employer failed to ensure that employees manually removing rebar mesh from the MG215 (Typ 32.14/16D Nr GS1.4174.5622 & Typ ZG 32 QKVI/1 Nr GS1.4516.5622) were protected with machine guarding methods. At times when the MG215 timed out due to an error in the programming, and/or the hooks missed when picking up the rebar mesh to advance it, employees were climbing onto the Typ ZG 32 QKVI/1 Nr GS1.4516.5622, to gain access to the wire rebar mesh to remove the mesh by hand without being protected with machine guarding methods.
