29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements: a) Heavy Equipment Shop back storage room: The right hand ascending side of the stairway used to access the mezzanine storage area, which had 14 risers, did not have a stair rail system, on or about 8/11/17. b) Heavy Equipment Shop back storage room: The left hand ascending side of the stairway used to access the mezzanine storage area, which had 14 risers, did not have a mid-rail, on or about 8/11/17. c) Yard Fueling Station: The left hand ascending side of the stairway used to access the top of the On Road fuel tank, which had 12 stairs, did not have a mid-rail, on or about 9/8/17. d) Yard Fueling Station: The right hand ascending side of the stairway used to access the top of the On Road fuel tank, which had 12 stairs, did not have a top rail or mid-rail, on or about 9/8/17. e) Yard Fueling Station: The right hand ascending side of the stairway used to access the top of the Off Road fuel tank, which had 17 stairs, did not have a mid-rail, on or about 9/8/17. f) Yard Fueling Station: The left hand ascending side of the stairway used to access the top of the Off Road fuel tank, which had 17 stairs, did not have a mid-rail, on or about 9/8/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
