29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: a) Koppers Conveyor - Timbers used as energy isolating devices to support a raised scissor lift conveyor did not effectively isolate the gravitational potential energy to prevent the conveyor from falling. On July 15th, 2017, an employee suffered fatal injuries when timbers used to support a conveyor failed, causing a scissor lift table to fall and crush him while he was working underneath it. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
