29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On or about June 22, 2017, at the jobsite located near 7727 Horatio Drive, Saint Louis, Missouri, the employer did not provide for adequate frequent and regular inspections of the job site. An employee sustained an amputation to the tip of the ring finger of the right hand while operating the IST Reel (Make: Ritec; Model: Motorized Cable Reel XXL; S/N: 81) with a missing guard that went unidentified by the designated competent person.
