29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) 650 South State Street, Dover, DE - On or about July 5, 2017, the employer failed to ensure an orderly and safe shut down of equipment prior to servicing or performing repair and replacement of the actuator located on chiller #3. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], stored energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to safeguard employees from the machine servicing hazards.
