29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy controls: a) Ames True Temper Inc., Mill Area, on or about 6/22/17: On 6/22/17 an employee suffered a pinky amputation while troubleshooting the limit switch that activates the jump chain transfer. The jump chain energy control procedure did not include steps for isolating and testing the jump chain transfer limit switch to verify the effectiveness of the lockout.
