29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems (a) South of the office building: On or around June 15, 2017 the employer did not ensure a "competent person" inspected trenches and excavations prior to the start of work and as needed throughout the shift. Employees were working in the excavation at depths ranging from six to seven feet. Employees did not have the protection of sloping, benching and/or engineering controls such as a trench box. Inspections were not completed for the potential cave-in hazards on the site. Note: Abatement certification and documentation is NOT required for this item. Corrected during inspection.
