29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) 405 Park Drive, Carlisle Pennsylvania, Butter Print Room - An employee changed an photo eye on a case packer. The employer did not ensure that the employee placed a lock on the case pack out prior to performing the work , on or about June 16, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
