Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and shall be done in the following sequence: a) On or about June 6, 2017, maintenance employees were exposed to amputation injuries in that the transporter conveyer in the Singer Area was not locked out prior to cleaning, in that an employee received a partial finger amputation while performing troubleshooting activities. The employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to maintenance tasks. The employer did not: - Turn off or shut down using the procedures established, - Physically operate energy isolating devices to control energy sources and affix lockout device(s), - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe, - Verify that isolation and de-energization of the machine or equipment had been accomplished. b) On or about June 14, 2017, maintenance employees were exposed to amputation injuries in that the transporter conveyer in the Singer Area was not locked out prior to changing the belt. The employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to maintenance tasks. The employer did not: - Physically operate energy isolating devices to control energy sources and affix lockout device(s), - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe, - Verify that isolation and de-energization of the machine or equipment had been accomplished.
