29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Fire Service Safety, Inc. located at 628 Township Road 95 in Huntsville, Ohio; On or about June 2, 2017 the employer did not ensure that a home constructed man-basket that was lifted via a Komatsu Forklift (Model #: FG25TLP-12, Serial #:515772A) to lift an employee in the air to perform power washing was approved by the manufacturer. Employees were exposed to fall hazards in the event the forklift or the basket would lose stability.
