29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and shall be done in the following sequence. a. Crown Equipment Corporation located in New Bremen, Ohio: On or about May 22, 2017 the employer did not ensure employees using a coolant hose to spray metal chips from the cutting area of the Amada HFA 400W horizontal bandsaw, locked out the saw and material clamp prior to performing the cleaning operation.
