29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: On or about April 21, 2017, and in times prior thereto, workers were elevated on a Sky Trak equipped with an unapproved, unstable shop built personnel basket. This condition exposed workers repairing and replacing worn portions of the rappelling surface to a fall from elevation hazard.
