29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about April 7, 2017, an employee operating the 5 foot panel welder was exposed to caught-in and amputation hazards while attempting to place a moving wire back on a straightener roller and the employer did not ensure that all energy isolating devices were operated in order to isolated the equipment from the energy source.
